Amergis Direct Support Personnel in Laconia, New Hampshire

Amergis Healthcare Staffing is seeking a Direct Support Personnel. The Direct Support Personnel assists with growth and development of interpersonal skills and Activities of Daily Living in the home or community to individuals with MRDD or mental health challenges who live independently (with assistance) in their own residences (supportive living) or a community residential property.

Essential Duties and Responsibilities:

  • Provides training/assistance to assigned individuals with Interpersonal skills and Activities of Daily Living in accordance with the outcomes and goals in the individual’s support plan and under the direct supervision of the Team Leader, Program Coordinator, or Program Director

  • Addresses all outcomes and goals as outlined on the individual’s ISP and BSP are performed

  • No changes in care are initiated without direction from the Team Leader

  • Any changes in status are reported to Team Leader, Program Coordinator, or Program Director

  • Documents care per company policies and procedures, evidenced by notes are completed, accurate and turned in on a timely basis, and in accordance with documentation policy guidelines

  • When unable to complete an assignment, for any reason, office leadership is informed immediately. Changes in assignment due to consumer or family request must be verified. Amergis Healthcare Services must be aware of changes in assignment and reason for change. Confirms schedule weekly with Team Leader/office leadership

  • Participates in Multi-disciplinary Team conferences and In-services as requested, verified by sign-in sheets

  • Mandatory In-services are attended, verified by sign-in sheet

  • Performs other duties as assigned/necessary

Minimum Requirements:

  • High School diploma or its equivalent

  • Acceptable motor vehicle driving record

  • Reliable transportation

  • Current state driver’s license (or state ID if not driving consumers)

  • Current automobile insurance

  • Proof of 2 references or per state requirement

  • Current CPR card per state / client requirement

  • Current TB skin test per state / client requirement

  • Proof of six months MRDD experience preferred

  • First Aide certification per state / client requirement

  • Ability to effectively elicit/provide information to and from appropriate individuals (including, but not limited to, supervisors, co-workers, clients) via strong communication skills; proficiency in the English language may be required

  • Computer proficiency required

  • Must be at least 18 years of age
